Health forms

Who sets this up: owner or manager.

Health forms (anamnesis) record important information about the client — allergies, previous treatments, skin or scalp conditions.

Create a template

1. Menu Settings → Health forms.
2. Options:
   • Use a ready-made template — hair, facial, body or general (an editable copy for your studio)
   • Create from scratch — build your own questions
3. Give the template a title (e.g. "Hair form").
4. Save.

Ready-made templates

Adatis provides base templates by specialty:

Template	Best for
Hair / chemical	Hair salons, colouring
Facial aesthetics	Facial cleansing, facial procedures
Body	Massage, hair removal
General	Any area

When you use a ready-made template, Adatis creates a copy just for your studio — you can edit it without changing the system default.

Fill in a client's form

1. Open Clients → select the client.
2. Health form tab or section.
3. Choose the template and answer the questions.
4. Save.

Who can view

The owner, a manager and reception can view and correct completed forms. A professional can view all of them and correct the record they saved. See Correct a health record and Roles and permissions.

Tip

Start with a ready-made template for your main specialty and adjust the questions after a few weeks of use.
